Solid Ground Permanent Housing Application Process

Complete application completely and accurately

Submit copies of the following with the application:
Social Security Card
Picture ID or Birth Certificate
Completed Police Pass

Once the applications are reviewed, applicants will be notified to begin the interview process and should be prepared to supply the following:

• Employment History
• Previous Residence History (where, when, how long, rent paid)
• Special Health Concerns
• Documentation of Disabilities
• Basic Knowledge of Goals They Are Trying to Reach
• Names and Contact Persons of Any Agencies Providing Services
• Release of Information to contact these persons
